Patient Forms & Office Information
Your first visit to Wythe Family Dentistry establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. To understand what to expect for your first visit to our practice, please read through this page. You’ll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. You can save some time at your first visit by printing out and completing the patient forms in advance of your appointment.
Your First Visit:
You can expect exceptional service at Wythe Family Dentistry. During your first visit as a new patient, we will review your dental and medical history forms, and take X-rays of your teeth and mouth. Then, one of our hygienists will clean and polish your teeth. Afterwards, your dentist will meet with you to discuss your X-rays, check your teeth for any cavities or early signs of tooth decay and make sure your gums are healthy. He also will conduct a thorough oral cancer exam. Depending on your needs, your dentist will personalize a treatment plan for you and answer any questions you may have. Payment for your cleaning and exam are due upon the time of service. Please remember to bring your dental insurance card with you to your first appointment.
Regular Check-ups:
The American Dental Association recommends visiting your dentist every six months for regular checkups and routine teeth cleanings to keep your teeth and gums healthy. We understand that you have a busy schedule, so when you’re preparing to visit our office, please remember that your appointments typically last 60 minutes.
